That’s definitely a blue stripe. 100/60W. Blue and greens were being produced simultaneously. Basically, all green were simuls. If you wanted anything other than simul-class you got a blue stripe, because the preamps are the same.
So 100/60W, 60W and Coliseum blue stripes were being produced...
Those are two EQF-2 units.
Well I haven’t actually tried running it in front of the amp. But technically I guess it would work. I’ll have to try to confirm.
But 100W and simul-class are two different versions that use different output transformers. The green stripe is simul-class 85/25W and should have the 562004 OT.
Yes that is my observation as well. Haven’t seen any cabs with 65’s with the jack at the top of the cab either. I haven’t seen any pics from the studio that shows the back of the cabs, but this pic of Kirk’s rig from the Puppets tour shows cabs with the jacks at the top.
According to Flemming there were around a dozen cabs in the studio that all sounded different. I find it unlikely that they shipped a dozen cabs from USA to Denmark. To me it’s more likely that the cabs used for Puppets were property of Sweet Silence.

If you by gain boost mean the pull function on the Master 1 knob that is actually the same as the pull deep. Gain boost is just what the old IIC faceplates said, the function is the same.
I’m pretty sure Lamb Of God used the rev B. At least 3 of 4 heads in this rig rundown looks like IVb, with the smoother tolex and the rounder EQ slider knobs. Only the top one in Willie’s rig looks like it could be an A, but it could also be an early B.
I’ve had both, but prefer the A myself.
