    When I had one of these it needed an iso transformer between it and the amp, at least in my set-up. I guess that's why the later PQ4 had a ground lift switch added, and it's a pretty common issue with using mains-powered stuff like that.

    Sounds like you´re picking up some outside interference - does it change when you move around the room or back away from the guitar? Might be as easy as turning off some device somewhere, or maybe you need to check the shielding in the guitar.
