Just total. You won't pay tax on the ones you made a profit on if your total is a loss. I would personally do it line by line anyway, but you don't have to if you know all your total revenue and expense.

I've definitely had squealing like that with multiple Marks. There's settings that don't work beyond a certain volume level, especially if you're that close to the amp and/or running high output pickups.
Yep agreed. Extreme mode is meant to mimic the IV with Presence pushed and the IV mode is presence pulled. The VII is way closer to the IV though.
The Mark V really is a good amp though when you consider all the things it's trying to do and that it's 15+ years old at this point.
