Need help finding a new Mesa tremoverb footswitch

Anyone have any good solutions for a new footpedal to work my Tremoverb head? would like a channel switch, reverb switch and tremolo swtich on the same pedal. Thoughts?

Amp on left in the wooden headshell. you can see the input and the foot switch jacks on the far left:
