There is a guy selling these on Ebay from the Netherlands so I took a chance and bought 2 of them. These are very nice with very dense plastic and the dimensions are a 100% perfect fit to the original, I did not have to do any sanding or drilling. He ships these with 1/4" neutrik jacks but the standard 1/4" Cliff jacks fit just fine, which is what I already had wired in my cab. Took about two weeks but they showed up with no extra fees/tariifs/customs fees and are quite nice. I'm suprised no one in the US is making anything like this.

I like having parallel outs so I just use the original plate and wire it with 2 new jacks and then put a small piece of weather stripping over the middle switch hole.
