I play mostly with the thumb almost up against the 6th string.

I think this is the root of the problem. Recently been playing around with thumb position due to big stretches (my requirement was 3h7 using index and ring fingers)

moving the thumb down from the wrapped position allows your hand to stretch between fingers more easily. In my experience it was the thumb wrapped position that caused the most wrist pain when trying to make those big stretches.

Yeah there's no way to do that stretch with the thumb like I've always placed it in the past. It's just really weird playing with the thumb under the middle of the neck and the wrist all crunched up. I can play it, just not at 299 bpm like they do. Like I said, I don't know if it's worth it to spend 100 hours on one damn riff, but I suppose it will open other doors if I stick with it.

For me it is a combination of guitar height, thumb position and wrist position. If I sling it too low it takes away from my reach ability. For me the guitar has to hang to where m pick hand picks right at bellybutton level.

I never use a strap, I'm usually on my ass. I don't play out, so why bother standing up.
