Not a small builder necessarily but I've had Schecter in California (Burbank?) make a few one-off customs. They nailed it; made sure to ask thoughtful questions along the way to make sure it'd be right and tested some things to make sure they could do it.
I'm not sure they'd make that specific shape and headstock if there are trademark concerns, but it'd be worth asking. I placed orders through Jason at Drum City Guitarland but you'll likely end up connected with the shop itself for discussing the details.
