Its a Voodoo Man 4 button but I don't think he's making them anymore. Loop > Channel switch > two channel boost buttons.

A guy on TGP was able to recently get one from somebody else, go to this site and search for Wizard foot switches:

Here's a shot from the TGP thread with the new pedal. If you have an MC25 remember to get the leads to the bright inputs appx 12" longer; if full size MC type just get regular
