Maybe need to file some more. Plating could be a little thick on the inside. Hard to see in the pics. Take some scotchbrite to the bolt also.

What part should I focus on when filing? Just the section that holds the bolt?

The whole inside. Especially where you see that circle from the casting and the length of the forward narrow section.
I had one that was especially stubborn. I thought I filled enough but went back 3 more times before it worked right.
Last one I bought worked great on my evh right out of the box.
Patience you'll get it.

Is it an older Axis or one of the newer Axis Tributes? I ask because a number of people including myself have complained about difficulty installing the D-Tuna correctly on the Axis Tributes. They have a re-designed Gotoh which could have messed something up but I'm not sure what. I have a number of Axies, on all of the older ones there is zero problem, on my Tributes only one of them works half-assed and I gave up trying to install the D-Tuna on the others. Some point too far down and one of them only gives me a half step like you are saying. Weird and annoying.

The first thing I noticed in your pictures is that you need to use the longer of the screws provided in the D-Tuna package. It looks like you are not clearing the back of the floyd with the D-Tuna. It could just be the way it appears in the pic, but it's just something that I noticed.

I have one of the older Axis. Which is the weird thing, I'm thinking it's just a defective D-Tuna. Going to try and exchange it and see if that does the trick.

I've tried both screws multiple times  Thinking the issue is with the D-Tuna itself..
