I thought I read that Scott Splawn himself said the B+ and old switch cannot be retrofit, but the mid cut is an available retrofit option for older amps.

Hmmmm... when I talked with Scott he said he could do the "Old/New" on my 2013. Here are his exact words from his board.
"Hey Guys
The mid switch is the only option that we could do on all the amps we have built. The Drop B+ switch depends on the power transformer that is in the amp and the old/new switch has to have the latest PCB, some 2014 and all of 2015 would work. When you switch to the Drop B+ you will not have to change the bias, the switch takes care of that."
